This channel provides reliable and focused patient education content at 5th to 6th grade literacy level. The programs are designed to encourage patients and families to be actively involved in their health care treatment and recovery plans. This will help staff meet regulatory requirements for patient education and care management. A Patient Channel table tent and card for each patient room will be delivered shortly. This will instruct patients on which channel to turn to. Quarterly, new program schedules will be distributed that can be provided to patients. Staff should encourage patient’s to view these programs and document this education. The Patient Channel Website is available which can also assist the staff in utilizing this system for our patients.
